Community Reintegration Project
Lake County Center for Independent Living can assist individuals with disabilities who:
- Are currently residing in a nursing home.
- Want to move to their own home.
Services include:
- Information on accessible and affordable housing options.
- Financial assistance with the cost of moving back into the community.
- Assistance in locating Personal Assistants.
- Independent Living skills training.
- Information and referrals to community agencies and other support services.
- Information on assistive technology and devices.
- Determining eligibility for DRS Home Services.
- Advocacy.
- Benefits counseling and referral.
- Peer counseling.
